Net Nutrality -VS- Pay per Byte

I'm sure there is some subtle difference here between net neutrality & the old ISP notion of paying for how much is used.  Vonage & others have a nice business model of a flat fee for use of their phone service.  Comcast currently has a flat fee for bundling the 3 basic services of Cable TV, Internet & broadband phone service.  If they start charging different rates for different usages I suspect the one ISP that doesn't might get all the business!
